In order to participate in the first day of tryouts you will need to have the following:

1> Consent Form

2>    Physical form (1 physical for 4 years of High School and 1 for 3 years of Middle School)  

** Physical  form must be filled out by physician (or copy of sports physical on file with school nurse)

3>  Proof of insurance (Please provide a photo copy of your child's insurance card.)

Even if you have filled these forms out for another sport, please take a few minutes and do so again to insure your child’s safety and medical information is current.
